VOLLEYBALL TRAINING WAIVER AND RELEASE OF LIABILITY.
THIS IS A LEGALLY BINDING DOCUMENT. PLEASE READ CAREFULLY BEFORE SIGNING.
By agreeing to this waiver, I, the undersigned parent/guardian of a minor, voluntarily agree to participate in volleyball training sessions organized by Focus Sports Academy Corp and acknowledge understanding of the terms herein.
● I understand that volleyball training involves inherent risks, including but not limited to: - Injuries from collisions, falls, slips, or contact with equipment/facilities.
- Strains, sprains, fractures, or more severe injuries (e.g., concussions, cardiac events). - Risks associated with facility conditions, weather, or equipment.
● I voluntarily accept full responsibility for these risks and waive any claims for injury, illness, or property damage arising from participation.
● I hereby release, discharge, and hold harmless, Focus Sports Academy Corp, its staff, coaches, volunteers, affiliates, and facility owners from any liability, claims, or demands arising from participation in training, including negligence, to the fullest extent permitted by law.
● In the event of an emergency, I authorize the Organization to seek medical treatment for my child. I agree to cover all associated costs and release the Organization from liability for such decisions.
● I confirm that my child is in good health, physically fit to participate, and have consulted a physician if necessary. I will notify the Organization of any health changes that may affect participation.
● As the parent/guardian of a minor participant, I agree to all terms herein and release the Organization from liability on behalf of myself and the minor.
● I agree to follow all instructions, safety guidelines, and rules provided by the Organization during training.
● I grant the Organization permission to use photos, videos, or likenesses of myself/my child for promotional purposes without compensation.
If any provision of this waiver is deemed unenforceable, the remainder shall remain valid. I certify that I have read this waiver, understand its contents, and agree to it voluntarily.
